TenuringThreshold being adaptive when UseAdaptiveSizePolicy is off

Tao,

CMS no longer use InitialTenuringThreshold.
IIRC, even before when CMS used InitialTenuringThreshold, setting initial
and max the same value didn't turn off the adaptiveness.
