Looks good.

One thought about related code:
    while (_iter_table != NULL) {
      MutexUnlockerEx mu(CodeCache_lock, Mutex::_no_safepoint_check_flag);

We might want to consider changing this into using wait/notify instead 
of unlocker/sleep.
